Many people who say they will pay cash do not arrive for an appointment.  If you would like to pay cash, we will make appointments when we receive $100 cash or debit card.  That will be unrefundable. At current writing, the fee for cash at initial appointment is $220, and for medication management  follow-up appointments, $110. 00

Dr. Elder is on their "panel", which means that an agreement to wait for insurances’ payment or rejection of payment has been made.   Please call the number on your card and:

Check that your co-pay is indeed $20, as it is for almost everyone with this policy. Have you met the deductible yet? If you've been seeing other therapists, ask how many sessions you have left for the 2-year covered period.

Dr. Elder is on their "panel", and they use Pacificare above to handle everything.  Please call the number on your card and ask all the Pacificare questions.  Have a magazine handy; they may transfer you to Pacificare and have you start the phone tree game over again.

Dr. Elder is a "preferred provider".   Please call the number on your card and ask what percentage your plan pays, if you have met deductible, and  what percentage you will co-pay (I'll ask you to pay that amount each session, please). Have you met the deductible yet?.

On panel.  HMO plans require a co-pay.  Please call the number on your card and ask:

· How much is your co-pay?  Have you met the deductible yet?

· How many sessions do you have left for the 2-year covered period?

· How many sessions before Dr. Elder needs to submit a document to "re-authorize"?

Thanks.

Unless you are able to hard talk these people into a “single case agreement” on my terms, you will have to be a cash patient, as in Cigna below.

You will be asked to pay Corvallis Psychiatry at the time of service and then present a receipt .  Please contact them now and ask how they will reimburse you, and just how to submit receipts.   You should call Cigna  to arrange a “single case agreement”.    They usually will arrange for a reasonable contract for your visits with me.
