SERVICE LIST

 Initial interview:                 CPT code 90801, one hour             

      See “Psychiatric history quest” and “Symptoms”.   Prior to this meeting, please fill these out and submit “release of information” to prior practitioners who have details about medications and psychotherapy treatments.  This will assist in making the process move quickly and smoothly toward solutions to the sometimes difficult problems you may be bringing.  Please call your insurance provider, make sure you are authorized to see Dr. Elder, and find out what your copayment and deductible will be. 

 Psychotherapy                    CPT code 90807, 45-50 min                

This is individual work, most often with CBT, (cognitive-behavioral therapy) approach. 

 Medication management      CPT code 90805  20-30 min                    

This is focused on medication management: components are 1)Status of symptom targets  2) Status of side effects 3) Customer satisfaction and continued plans.

   

 

 

Evaluation and treatment

Group treatment of anxiety or depression is highly effective, less costly, and can even be fun.  Consider being a part of the group by referral from your therapist or doctor, discuss with Dr. Elder at initial interview.  See “Group link” for syllabus.
